- The proxy must be a member of SMDPCCU in good standing and must bring valid ID to the meeting.
- Deliver the completed form, with a copy of the member’s valid ID, to the Secretary at the Head Office or any branch at least 48 hours before the meeting starts.
- If the member attends the meeting in person, this proxy appointment is cancelled.
- Proxy voting is subject to the Bye-laws of the Credit Union and the rules set out in the official Notice of the Annual General Meeting, which take precedence over this form.
